Early in the shuttle program, the explosive bolts that attach the solid fuel boosters were coming back a gouging the main tank.
The solution was to manufacture “Explosive bolt capture box” attached to the main tank. The object could be one of those bolts.

Sure looks like it. Something left behind by a tech working on the space craft. Maybe some foil from a shroud or ice buildup from some vent on the craft.
