Does 104% mean more votes cast than those registered to vote? Were more votes cast at polling place than those recorded as showing up on election day?

Yes, more votes than registered voters. Has to be voting officials adding ballots because they check off names of voters as they vote. When Allan West was defeated in Florida the turnout was a whopping 140% or so.
