Belligerence is not a crime, but it gives the officer a reason he can articulate to explain why he ordered the person out. “Her belligerence heightened my concern about my safety and I felt I needed to have her exit the vehicle to ensure my safety.”
Again, he doesn’t even need a reason. “Lawful traffic stop” is all the justification he needs to order any or all occupants out of the car. She could have been the sweetest, friendliest, most politr person he ever pulled over and he could have ordered her out.

Yes. Had she not thrown a hissy-fit over what was to be a simple warning citation, she would still be alive.
A brief anecdote I know from the workplace: a guy who was harassing fellow workers online in a chat room was called in by the company he worked for for a "psych test". It was strictly harassment on the part of the company. His union advised him to go to the appointment, because at that point if he didn't show up he could be fired. He was very hardheaded though, and didn't show up. He was fired, and lost his appeal. I don't know what happened to him after that, but he didn't ever get reinstated.
