In France, this isn't a problem.

In the middle 50s, my ship was in the Mediterranean and I got a special week's leave to do some touring.

At a reasonably classy restaurant in France ... near the German border, there were two restroom doors next to each other, one for "Madames" and the other for "Messieurs."

Then, while standing at a urinal, I noticed (over my shoulder) someone entering; a woman ... heading for a toilet closet.

Looking back then to the entrance area, I saw that both doors were for that one room.

There is a very simple solution to this, thought a bit graphic.

Restroom doors should have an anatomically correct silhouette on the door.

Otherwise, each building will need 6 or so different restrooms: One each for gay men, straight men, lesbos, straight women, bisexuals, & trannies. Down the road, building will need to add one for pedophiles, beasty boys, voyeurs, etc.

My sign suggestion resolves ALL of that.
