It is possible to be deficient in B,C,D,E and Omega while eating a junk food free diet. Aging, prescription drugs, smoking, alcohol, environment etc as well as not eating enough of the foods that supply these nutrients can cause it.

That they didn't eat what we call junk food doesn't mean they ate nutritionally beneficial foods.
