Not “no”, merely few. Toilets with running water existed in Roman Empire times.

Outhouses are underrated, anyway. They were considered valuable during Civil War times, and not just for their relief factor.

In North Korea, there were quotas for city dwellers to deliver their euphemisms to the appropriate bureaucracy for use as fertilizer. Dreadful if one were “irregular,” I’m sure.
