actually, i tend to not think of that unless its brought to my attention.
however if i saw a person at mcdonalds, double fisting big macs, mawing on a superzied order of fries, my instant reaction would probably be “fat” followed by some colourful metaphor.

Our ice-age evolution made fat storage a VERY beneficial factor. It's probably difficult to dump something that allowed survival in tough times.

Cut off the food and it's obvious that a "fat" person will be alive and busy long after all the thin ones have died of starvation...
