Yes, the pill is 98-99% effective, the condom only about 80% effective IF used correctly. Unfortunately, young adults don't take the pill every day at the same time, so it is less effective. As for condoms, they break, they may not be used every time, and often men put them on after they have released pre-ejaculate fluid that contains sperm.
At our Crisis Pregnancy Clinic, we have a lot of clients on the pill that get pregnant and even more that use condoms. That's why teaching "safe sex" is not effective.
Talking about a 44 year old woman who became pregnant. I am aware of the stats for young adults. The question of judgment enters when Sarah Palin chose to become pregnant or simply was negligent. Either one speaks to her judgment.
BTW not that it would matter in the case of an adolescent, but the numbers I have seen as to the effectiveness of a condom used (as it always should) in conjunction with a spermacidal gel is 98% as well.
