My suggestion would be to increase the incline and the resistance a little bit and go as hard as you can (it sounds like you're doing a "hill" program?) until your body gets used to both...then increase both factors little by little as your body gets used to the increased stresses.

I say this assuming you have no life-threatening conditions and your doctor has cleared you to work out.
