I see "signature of applicant".....but would both parent's have to sign?

I believe now that is a search for a certified copy of an original birth cert.

The file is here and looks like anyone can request a copy of the birth cert.

Instructions:
Complete application form
Hand in completed form and wait for name to be called verifying search completed
Once your name is called take play slip issed to #50 Shirley Street to pay for service
Giv receipt to Births and Deaths staff showing proof of payment and receive certified copy of birth certificate.
