I have found what looks to be the training manual for the SANE program in its entirety......it includes the exam forms used to comprise the "report".....this is a 299 page pdf file, and the forms begin on pg 203...I saw Minnesota and Tenessee on some of them, and one can speculate as to whether the forms in this case are identical except that they are letterheaded with Duke University Medical Center.....nonetheless this document allows a better understanding of the entire "SANE" matter in this case.
