Yeah, it is on the Season 4 DVD. It explains how someone figured out Jack is alive.
Basically, it takes place in Chicago. Jack is living in pretty bad conditions. He is waiting on someone and notices a person on a motorcycle watching him. Chloe arrives and they talk for a minute. She tells him that someone hacked her system and accessed all of the info on his autopsy. Chloe fears that the person who did it will figure at that Jack is alive as they were so good that Chloe couldn't trace them. Then she says "I can't believe you have to live like this" (Picture 5) and is clearly hurting for him. They depart, Jack asks her how Kim is and Chloe says she was devastated but is better now. Jack takes the cell phone Michelle gave him at the end of Season 4 and trashes it.
As he prepares to drive off, a car comes up behind him and the chase is on. After a car chase, he manages to dispatch the bad guy in typical Jack fashion. He drives off with the guy on the motorcycle still watching him.
There is also a prequel for Season 4 (on the Season 3 DVD) where Erin Driscoll fires Jack and he goes to work for DOD. As he walks out of Driscoll's office, she offers to help him find a job. He replies with "I can find my own f***ing job!"

That's one of the deleted scenes in the Season 4 DVD. The looks on her face alone are PRICELESS!!!
Could help but notice in the prequel that Jack and Chloe were driving 2007 model Toyota's. I guess they're not "conservative" enough since that isn't an American car.
Also the "conspiracy" piece was neat to follow along to.

I did see that at the beginning. No knocking the choice of vehicle as a matter of fact it the Camry was my runner-up choice of car when I turn my Buick in in a month. I'll be tooling around in a Nissan Altima soon. It is my first foreign car purchase.

I don't think she was nervous, I think it was part of her weird/neurotic/quirky act. It was a really funny interview, especially when she was like, "I'm dating my therapist, but he makes me sleep on the couch. And I think he's seeing other women." Hehe.
