With a pad like that, you put a regular square or contour pad underneath it.

I like to have some kine of thin square blanket underneath my pad, just because it takes care of most of the dirt and I can just throw it in the washer every time I use it and I don't have to wash the pad. It also keeps my saddle bags cleaner. I made this one out of black felt material that I bought at Wal-Mart. It took about a yard I think. I folded it in half, stitched it around the edge, then threw it on the horse's back and pinned it to the contour of his back from withers to hip, stitched it following the pins, then cut off the excess. That way it lays flat and doesn't bunch up under the saddle. I made the first one out of polar fleece but it didn't have enough stiffness and kept creeping back under the saddle so I tried the next one out of the felt and it was just right...
