Heart cases don't wait in the emergency room, even when they're without insurance. I have had heart surgery four times, I speak from experience. Some of those times, I had excellent insurance, and some times none at all, and no money to pay. All they care about with heart patients is keeping them alive, getting them stable, and getting them fixed up. They let the bean counters worry about collecting the fee.
Having chest pains? Going through a rough patch in life? Doesn't matter. America's medical system has always shown me it's best side. If I manage to die a millionaire, my inheritance is going to go to building a hospital for heart surgeons to learn in. I cannot say enough good about them.
