Regarding the treatment: I've really been wondering about that. Is there more standardization now? Less intubation right away? Are the obese housed in seated positions, or lying on their sides? It does seem the medical care at first was utterly chaotic.
Will we ever know the answers?

Except the over reports are 1600% That means about 35,000 died off the top of my head. Using over reported fake numbers leads people to make wrong conclusions and over estimate their risk.

I don't disagree with you that the vaccines certainly seem to be working...I have my second shot coming up soon, and I hope it helps rather than hurts.
But to suggest that a "reputable" company wouldn't fake results is to ignore breath-taking conjobs since the origin of trade.

Minor cold symptoms - so who cares? Look at the trials reports on severe symptoms. The number of instances are more for placebo, but both arms are minuscule. Scale matters.

Outside of my professional wheelhouse but from what I can gather, yes! When our 'thought leaders' were screaming about more ventilators Trump ran out and forced new production. By the time they came online it was apparent that 80% of people put on them died. Not to say the ventilators themselves were the problem but certainly evidence of zero to negative effect.
It is my understanding they are using, in the most severe hospitalized cases, a regimen of intravenous anti-virals, steroids, fluids, vitamins and minerals as one might use to fight any viral infection or severe flu. But you'd have to check, I think there is still no standard protocol used universally and that's the fault of the CDC/NIH (Fauci!) who bet the farm on vaccines and spent next to nothing on treatments throwing everyone under the bus for a year.
I'll add there is more awareness of and use of ivermectin (and maybe HCQ+ therapy) earlier in the disease cycle, again after a year of a massive institutional push to try to classify these very old, very safe, widely used medications (both of which are classified by the WHO as essential medications for every country to have stockpiles) as some kind of danger to humanity.
The trials did not rely on PCR tests, (which you sometimes call PRC). So this whole post is not relevant. They are based on comparing those vaccinated against unvaccinated controls, for symptoms, hospitalization and death, as well as test results (which does include PCR tests, as well as antibody tests)

From exposure to virus in the wild, like everyone else (including the control group).
Those vaccinated are dramatically less likely to develop COVID symptoms, be hospitalized or die - independent of PCR testing.
Those are the metrics of effectiveness.

Since then, we have been progressing through the more populous risk groups, working down the priority list. 70% of Americans over 65 years old now have at least a first shot. Although cases have levelled off in recent weeks, the death rate is declining faster, as the vulnerable are increasingly removed from risk through vaccination.
The vaccination effect is seen even more clearly in Israel, which is 2-3 months ahead of us, in vaccinating their population.
