Did Obama identify himself as a U.S. Citizen or a Legal Resident when he filed for his SSN?

Could you please point out where on the SS-5 application a person identifies himself as a Citizen vs a Legal Resident/(Green card holder) ??
Or would they have used a DIFFERENT version of this form if it had been an American Refugee Organization (ARO) based in Connecticut that had filled out the SSN application on his behalf?
I am convinced something on his application would be dreadfully embarassing (if not worse) to Obama, ...but what?
