Normally the self-subscribing oath is enough documentation but if the application is contested the election official can request additional proof of the claims on the voter registration application.

The BC would take care of citizenship and proof of age... BC not baptism certificate, although baptism cert once was OK to use for proof of age for driving licences at one time... a friend of mine used hers for age verification for D/L.
Not sure if baptism certs were OK for proof of voting age.
But BO may not have a baptism certificate from when he was a baby.
