Why not an opt out on abortion grounds?

are they actually allowed to ask what the religious objection is? In my state of Maryland, there are religious objection forms you can sign to opt out of routine vaccinations etc. but they cannot ask you information as to “which” religion you belong to, as that is private.

I’m thinking it would be very much the same on the federal level?
