Looking at he Smith Doc, I do not see and reference to Page or Book. Why? Because this is a HOSPITAL generated document. Hospitals usual record these things as standard medical records for the patient, in this case the mother giving birth to a... OTOH, on the ORLY doc there is reference to page and book. Why? Because it is a government form. Yes that is why there is no conflict in the names given on both:

(We’ve been able to pretty well varify the above. Others
