There has to be a way to make sure that does not happen ever again. The worker that goes in can lock the door open (both ends maybe) and gets the ONLY key.
There are multiple ways. The OSHA fines were because those ways were not followed.
I forgot to mention the padlock and keys in my pocket.

Over time, I’ve come to believe the tag is as important, and perhaps more important, than the lock.
Sounds like a confined space entry violation as well.
I hear some well-disciplined places make do with just the tags. The principle is clear: if you didn’t put the tag/lock on, you don’t take it off.

My most sincere apologies.
There becomes an eventual problem with that. It requires investigation by a supervisor, but an untagged lock, that the system has been walked down and inspected, is a big problem when it is time for production to get going.
Electricians have forgotten to take off locks. I've been a sites where a untagged lock has been there for weeks and no one knows why or who. You have to have a procedure for removing locks left unattended without tags.
My tags always include my cell phone number.
That was part of the OSHA fine, not identifying it as a confined space and treating the entry that way.
