WESTERN PACIFIC OCEAN (NNS) -- Two F/A-18 Hornets from Carrier Air Wing 17 embarked on the aircraft carrier USS Carl Vinson (CVN 70) crashed at 5:40 p.m. local time, today, while operating at sea in the western Pacific Ocean. The initial report is that the two aircraft are assigned to Strike Fighter Squadron 94 (VFA-94) and Strike Fighter Squadron 113 (VFA-113).
One pilot was rapidly located and returned to Carl Vinson, and is currently receiving medical attention. Search efforts continue for the second pilot.
The search for the second pilot includes guided-missile cruiser USS Bunker Hill (CG 52) and guided-missile destroyer USS Gridley (DDG 101) and helicopters assigned to Helicopter Sea Combat Squadron 15 (HSC 15) and Helicopter Maritime Strike Squadron 73 (HSM 73).
The two F/A-18C Hornets have not been recovered.

I’m not sure that would apply to the captain of a carrier where you have an air wing. That wing isn’t permanently on the carrier. Can the crashes be traced to a pilot training error when the squadron was ashore?

I was just reading about the carrier...
during RIMPAC war games in ‘98 a 60’s era submarine from Australia was able to come within 300 meters and launch green flares.... “sinking” the Carl Vinson.

Depends on the circumstances. A full investigation is always run because sometimes it's the pilot's fault and sometimes there is a system glitch. We had a helo run out of fuel and crash. It turns out that there was an unknown problem with that model of CH-46 at the time - If you kept turning one direction in executing a search pattern, the gyro would precess. So, in this case, the pilot did exactly what he should have done, but at the end of the pattern found that he was no where near the ship.

I’ve lost several friends/squadronmates due to mid-air collisions during ACM and air-to-ground sorties as well as just running into each other at night while trying to rendezvous. It’s still a dangerous business irrespective of the sophistication of the aircraft involved.
The cause of the crash remains under investigation, but NBC News reported that the planes collided before going down.
Collision avoidance would be an interesting line of technology research
I don't know what you mean by "draconian accountability," but the pilots will be held accountable thru an investigation process, which will determine the cause of the accident. If one or both are deemed personally responsible, then their careers are in jeopardy. I would not assume it is just pilot error.
Is being investigated per reports. Basically nothing additional.
BTW, one pilot in my Air Group survived two mid-airs, one in the F-4 and the other in a F/A-18. In both cases the aircrew in the other aircraft were killed. We used to tell him he needed to move to a cinderblock house in the prairie and have someone push oatmeal under the door to him because he was completely out of luck for the rest of his life.

During research am finding F-18s experiencing a large number of crashes lately. Was caught off guard. Didn’t realize till researching was this many birds gone down.
It’s a dangerous job, for sure. US pilots get a lot more flying time than probably any other country on Earth too.
The carrier air wing boss and captain of the ship are sort of like co-equals
Maybe they were engaged in ACM and ‘merged’ the hard way.
Since I was OOD on the Independence (CV-62), I do know what the F I’m talking about and it’s a valid point.
I didn’t say that in this particular case that is what happened.
I said that it can happen when pilot’s hot dog - which they do on occasion and I have experience with it when they are overly high on themselves.
