It happens. When I was a kid my dad came upon a cycle accident where a guy ran off the end of a road at a T stop and hit a single strand of barbed wire. Same result.
BTW, the helmet probably wouldn't have helped.

Henry was not wearing a helmet.
Somehow I don't think a helmet would have helped.

I lived on a canal in Grosse Ile, Michigan some years ago. A number of low bridges passed over the canal. The whole canal was marked as a no wake zone.
Some dumba$$ in a fancy speedboat went flying down the canal, up on plane. A neighbor yelled at him to slow down. The boater turned around and gave him the middle finger of both hands......and immediately hit a low bridge with his head.
Took his head off and ripped his spine out all the way down to the pelvis. All one big piece. They said he looked like a filleted fish.
