It happens. When I was a kid my dad came upon a cycle accident where a guy ran off the end of a road at a T stop and hit a single strand of barbed wire. Same result.
BTW, the helmet probably wouldn't have helped.

Earth First, they of the save the planet Sierra Club fame, used to sell a book on ecotaging off-roaders. It involved wire across trails at neck height of a seated dirt biker.

I had a collision at speed last year with a heavy piece of biological farm equipment on an FLHR. It was a sudden midnight orgy of blood and chrome and donkey shit. Left 20 yards of straight black rubber on the road all the way from visual to impact.
Better to hit those barrel ribs head-on than to risk high-siding my wife and I from a pair of deep shoulder ditches through barbed wire fences.
Condolences to the man who didn't make it in this story, that is a harsh way to go.
