That can be attributed to stupidity, not as a result (directly) of using marijuanna. Just like drinking alcohol and driving. Simply smoking it won't kill you. But you can die of alcohol poisoning if you drink too much.

If I recall the situation correctly, the operator of the train had reefer in his system. There was no evidence, however, that he had smoked just before going to work or that he was even impaired, given the fact that THC is fat soluable and can be detected in a person's system up to 30 days or more after use.
