I'm pretty sure that the last five-star was Eisenhower. It is not a "normal" rank that can be worked up to. Franks, Myers and those guys are at the top of the food chain and there's nowhere to go rank-wise for them now.
It is a specially created rank for unique situations, like the Supreme Allied Command for WWII.
