Right, that was Jon-Erik Hexum. Yeah, that's the guy. I was thinking of Brandon Lee, but I think he was felled by a "prop" revolver that was loaded with live ammo.

I did a search and Hexum was on the show, "Voyagers". He was doing a show called "Close up" when he decided to win a Darwin Award.
The explaination that was released for the Brandon Lee accident was that the gun was used in some close-ups and they were using dummy rounds that were put together with no powder or primers. They were hand assembled. When they unloaded the gun, Supposedly, with a capital "S", one of the rounds dropped a bullet in the chamber. Then, when the gun was used with blanks, the bullet was fired using the blank's discharge.
Brandon Lee was rumored to be a spoiled, pain in the $%^&. A very disliked individual.
