Upon whom should we declare war? What nation? What identifiable group of people (not some shadowy network)? Should Congress declare war "on evil"? And if war were declared on a definable nation or organization, when would we be able to know when the war was successful?

You left out Korea and Vietnam. Vietnam I can understand (but not like) how the DoW wasn't made because our involvement was gradual. The only excuses for Korea were 1. S. Korea was attacked, not the U.S., 2. It was a UN operation, not a U.S. operation.
I don't know enough of the history of the early part of the Korean War to know if a DoW against N. Korea and later China were considered and why they weren't made.
