"Carol of the Bells" is, I believe, the work of that most prolific of composers - "Traditional" ;)

I have a few versions handy - Canadian Brass, Mannheim Steamroller, Mormon Tabernacle. Those three are all pretty good.

I don't know, but it was used on the old Andre champagne TV commercials. When I was a kid, I thought it consisted entirely of the phrase "Christmas is here" repeated endlessly, but I've since learned there are other lyrics.
