LAS VEGAS (KLAS) — A Las Vegas grand jury indicted a woman for nearly $5 million of alleged Medicaid fraud.
Leasa Yvonne Carter is charged with three felony counts of submitting false statements for Medicaid fraud, perjury, and theft of $100,00, along with one gross misdemeanor of intentional failure to maintain adequate records.
Carter served as a Medicaid provider through ‘Square One Behavioral Health’ at two Las Vegas locations and billed Medicaid from Sept. 2018 to Nov. 2023 despite her Medicaid contract being terminated in 2014 for seven years, according to the Nevada Attorney General’s office.
Carter worked with Rickey Andrey Pierre, who had a criminal history, and Ariell Olivia Dix, who had also been excluded from Medicaid, to bill Medicaid, prosecutors wrote.
Medicaid would not have approved Square One’s Medicaid enrollment if Carter’s, Pierre’s, or Dix’s involvement had been disclosed, according to the indictment.
“Consequently, all of the approximately 47,601 claims submitted through Square One and all of the money paid for services allegedly provided through Square One, which totaled approximately $4,969,554.18, were submitted and obtained through concealment of a material fact,” the indictment reads.
Carter was also responsible for “billing as if individual providers were providing an impossible amount of services,” and billed the services under the names and identifications of providers who never actually provided the services.
Carter is also known as Leasa Yvonne Sanders, Leasa Yvonne Gonzalez, Leasa Y. Carter, and Leasa Sanders, according to the indictment.
Carter is scheduled to appear in court on Oct. 22.
Pierre pleaded guilty in connection with the Medicaid fraud through ‘Square One Behavioral Health’ and is scheduled to be sentenced on March 4, according to court records.
Dix also has a history of Medicaid fraud in Nevada and Arizona. She is currently serving a three-and-a-half-year sentence she received in May 2024 after pleading guilty to two felony charges of illegal control of an enterprise in Arizona. Dix helped facilitate Medicaid fraud by establishing fake clinics, according to the Arizona Attorney General’s office. Her projected release date is in May 2027.
Dix is also known as Ariella Olivia Dix.

Deloitte in CA and 25 states rips off both State and Federal taxpayers a lot more than the measly 5m of this provider.
Deloitte defective EMPI causes millions of dead people to be on the rolls as active. For 99.9% of these dead, a simple search of Legacy.com can show that they are dead and should not be costing the taxpayers money.
Deloitte defective EMPI causes millions of DUPS...the same person listed 2 or 3 times on the rolls as active...ie once as Sue Smith and once as Susan Smith...and maybe a third as Sue Jones-Smith.
How many other HHS contractors (and State contractors with Fed $) are ripping off the taxpayer?
(I personally would not be bothered that so many at Deloitte are H1b types if they were competent and saving the taxpayers money. But they are a waste of money.)
The important question is whether she contributed enough to Democrat party coffers to avoid an indictment for this long.
