If the “doctor” prescribed medication, that is the reason. Look at your diet and get back to real food.
It’s a test. One test determines nothing. It is amusing that you are prepared to accept the high scores and question only the low.
Your 86 looks like an outlier.
It could be something as simple as a laziness about storing memories, since you seem to have long-term memory deficits. It might be that your brain is so busy in the “here and now”, that minimal importance is placed on saving memories.
Maybe you can exercise your brain to do better with this by (for example), keeping a journal of a few prevalent memories from each day and reviewing these with some frequency and repetition. I think there was an article posted here on FR in the past day or two, that discussed this type of activity to improve memory.
To minimize the effect of ADHD, make sure you are getting 8-9 hours of sleep.
Have you ever been on ADHD meds? I was diagnosed in a similar vein at 40. The meds a ton of difference. Just might be worth a thought.
Was the long-term memory test the last test of the day? Maybe your brain was tired out by then.
I don’t question the low, it just doesn’t fit the others.
It all depends on what they are measuring.

Sounds like you’re working off super RAM but not enough pointers are being made to your ROM. Check your vitamin and mineral levels. You might just have some background stress. A relaxed state promotes faster ROM pointers.
Get tested for aluminum and heavy metals.
Keep in mind you can't out-think this. You will retrain your brain, working on focus and presence, in order to break the reflexive mental response that already has you in maladaptive state before you're aware of it. (Think of if you hear a car engine in the driveway, or the keys in the door - your mind already responded automatically before you were even aware of it).

I thought your short-term memory was OK. It’s so often you should have remembered that. I would get a second test to confirm.
have you had an MRI or a CTA to show what is going on with the brain, etc.
I am having memory issues. Mine showed an issue and a problem with venous outflow due to mechanical reasons. Some people with my mechanical issue have had good luck with Plavix but it comes with risks.
What do those long-term memory scores mean? “145, 143, 141, 139, 147, 151... 86???”
Too much alcohol or pot?
