I have questioned this too.
I don't think it was Crooks's shot. There is a video of Crooks staying completely motionless in the first couple of seconds after the first shot. The camera would have seen the recoil from the first shot if it was Crooks that had taken it. The first three shots very likely came from through the open, second floor window, behind and to the right of Crooks as he lay prone.
The would-be assassin cabal knew that in the early part of his speech, PDJT would be repeatedly referring the crowd toward the jumbotron. From that information, they could have inferred PDJT's head direction. But for they head-turn, they did well with that considered planning.
It was that God-guided twitch of his head that turned the back of his head away from the bullet's path, probably just a few of milliseconds after the shooter squeezed off a round.
Three tenths of a second later, PDJT's head left only his ear to be seen by the bullet.
