How do we know this breach in security was accidental?

The shooter was on a rooftop about 200 yards from DJT. It is inexcusable that that rooftop was not secured.

I disagree, any line-of-sight location out to several hundred yards should be secure.
They certainly will be next time. With Trump behind bulletproof glass next rally, perhaps.
