Children have been let down by a lack of research and "remarkably weak" evidence on medical interventions in gender care, a landmark review says.
The Cass Review, published on Wednesday by paediatrician Dr Hilary Cass, calls for gender services for young people to match the standards of other NHS care.
She says the "toxicity" of the debate around gender meant professionals were "afraid" to openly discuss their views.
NHS England says it has already made significant progress in making changes.
The Cass Review, which looked at gender identity services for under-18s, was commissioned by NHS England in 2020 after a sharp rise in the number of patients referred to the NHS who were questioning their gender.
It was announced after whistleblowers raised concerns about care at the Gender Identity and Development Service (Gids) - which was the only specialist gender clinic for children and young people in England and Wales.
Gids closed last week, four years after it was rated as "inadequate" by inspectors.

New guidelines by the health service say they will stop puberty blockers for under 18s.
'Transitioning' is all but stopped for under 18s and it's moving to more of a counselling model
The only "transition" clinic in England and Wales has been closed and a review found children were "let down" by it after it was found to be "inadequate".
However:
"Clinics overseen by NHS Scotland can continue to prescribe for children puberty blockers."
So Humza Yousaf can continue the policy :/

Most Children Grow Out of Gender Confusion Over Time, Long-Term Dutch Study Suggests
While proponents of âgender-affirmingâ treatment for adolescents claim that the confusion surrounding gender isnât just a phase that one can grow out of, a long-term, large-scale study from the Netherlands seems to suggest otherwise.
For their study, researchers at the University of Groningen looked at data collected from 2,772 individuals through a survey spanning 15 years. Beginning at age 11, the participants were periodically asked about how they felt about their own gender, until they reached their mid-twenties.
The survey contained a prompt, âI wish to be of the opposite sex.â The participants were asked to respond to that same statement every two to three years over the course of the 15-year study, and each time, they were given a multiple choice: âNot True,â âSomewhat or Sometimes True,â and âVery True or Often True.â
According to the findings, 11 percent of participants reported at least some degree of ânon-contentednessâ with their gender at some point during the study. But by the age of 25, the dissatisfaction rate had dropped to 4 percent.
I am glad she has delivered this report to the public.
This perspective is urgently needed in this country too.
But, it took her over Four Years to accumulate sufficient data supporting her conclusions? Seems that so much dysfunction would have been blatantly obvious after only four months of study.
The title and opening paragraphs tell little of the content. It’s almost as if the BBC were obscuring the truth

You don’t need “evidence” to oppose sexual mutilation of children. The mutilators are not going listen to any evidence that does not support their perversion.
Not how it works I’m afraid. To ensure even the gender affirming charities couldn’t dismiss it out of hand as being a biased opinion piece, it had to be forensically researched in all different directions.
I’ve worked on long running investigations - took six months on one of them to completely rule out cognitive bias.
Even England's socialist medical system has concluded that âgender affirming careâ in children is bad medical practice and should not be done.
