Wouldn’t the Senate have to convict Trump of an “insurrection” to keep him off the ballot?
It seems that would be double jeopardy since he was already acquitted once while still in office.

No, we are into the realm of a "political question" which SCOTUS doesn't like to touch. If the Congress declares Trump an "Insurrectionist" he could be barred from state ballots.
