You can have an errant mental obsession expressed as an identity dysphoria. It may not make you “insane” but it is a mental health issue, and issue in the mind, not an issue of your body.
To me entertaining the obsession is like an addict committed to a lifetime of getting what their addiction wants, instead of seeking the professional help out of the addiction.
