Rogue group releases a virus, some mad bureaucrat has this handy dandy delivery system he wants to try out for the promise of big bucks, gets the template for the spike protein from the same rogue group, prioritizes medicos and first responders for the initial rollout of the vax, a year later those first vaxed start dropping dead, then the next variant hits. Meanwhile, the rogue group who did not take that vax - they had their own - waltzes in. Gawd, I hope that’s fiction.
