Crappy pre natal care is not racist. There are so many avenues available for these women that giving birth to unhealthy babies is an effort.

Massive drug and alcohol abuse on top of poor nutrition, morbid obesity chaotic and unstructured life style.
Babies often have severe health problems including multi generational fetal drug and fetal alcohol syndromes due to all of the above, especially continued drug and alcohol abuse during pregnancy.
