Nobody's gotten it in decades, but you can submit the report right there. There's no verification of any information. Captain Picard of the starship Enterprise can submit a report saying the Pfizer COVID-19 vaccine killed Lieutenant Commander Data.

I thought you had real data from somewhere. Like this link from January 6th which states in part that "No deaths from anaphylaxis were reported after receipt of Pfizer-BioNTech COVID-19 vaccine."
