I earned my doctorate at a well known prestigious university. On my first day of grad school I was told that “the only ‘doctors’ here are in the hospital.” All professors, even the great names, were addressed as “Mister.”
I used to love listening to Doctor Dimento on the radio

BOL. I have a MBA. After I got it, I told my soon to be adult sons and my wife, they needed to call me Master! That didn’t work but once.
I have used that line with PhD’s wanting to be called Doctor. I informed them that would only happen after they called me Master, with respect.:)
Don't know if they still do it, but back in the day the higher qualified "Harley Street" physicians in England used the honorific Mister instead of Doctor. In Germany a physician cannot use the title Doctor unless he/she actually did finish a doctorate program (most do, of course).
