if its a ball point pen how is it suspended? usually pens have a part that is squeezed over a pocket and would be visible..

Maybe. What’s showing could be the catheter tail with cap. Maybe one of our doctors or nurses could weigh in.
I dunno what it is, but it seems odd he’d have any ‘ear piece’ equipment there. An IV? Mayybeee, a pen would also be kinda weird, but more likely I think. People don’t walk around with an IV much, and if there’s no bottle over his head with fluids, that really makes no sense.
