Washington State Attorney Stephen Pidgeon has said that he found at the Consulate a record for a name change from "Barak Mounir Ubayd" to ‘Barack Hussein Obama’ on 10/14/82 in Skookumchuck, British Columbia. The name change form, reportedly from the Ministry of Health Planning exhibits the Seal of British Columbia and the serial number 200415889.
