There were procedures and plans in place. The persons involved did not follow the required protocols. It was more of a case of human failure than procedures and plans. The FBI had a tip line, but the person receiving the tips never sent them to the Miami field office. The signs were all there.
Remember this 9/11, might have been averted if somebody would have listened to a loan field agent who had obtained intelligence that foreign Nationals were training to fly heavy Jets. The FBI knew about it and took no action to investigate it. The result - the Twin Towers came down. This is on a much smaller scale but its a tragedy none the less
We had a commission on 9/11 that identified all the failures. It was not as simple as you pretend. The point is that no matter how detailed and all-encompassing the plan, ultimately the responsibility falls on the people implementing those plans. In the case of this shooting, it was more of a human failure than a lack of procedures and plans. The people involved did not follow the plan or the procedures.

And the training, and the plans, and the procedures, all failed because they did not follow procedures. That’s a systematic failure.
When you don’t change your oil in your car and your engine fails it doesn’t matter if a crankshaft or a piston shattered it means the entire system as a car is now broken because it cannot move forward. That is systemic failure.
