I think that by definition it can only be awarded posthumously.

A lot of Darwin Award winners do so fatally, however, the rules only require that the winner be unable to reproduce.

In most cases it is. But the criteria is removing one's self from the gene pool, so an injury so severe, a self-sterilization rendering one incapable of reproducing also qualifies.
