That is what is more disturbing to me. If ships were being hacked or navigation systems were failing, etc., those are problems that can be solved by fixing technology.
Hacked or nav systems failing would potentially cause ships to run aground, NEVER run into another ship. There are lookouts whose entire job is to watch for other ships, especially small boats that may not be picked up by the radar system. And, for how agile our ships are, they should easily be able to avoid being rammed by a non-maneuverable cargo ship on the open sea.

This one is a bit different than the Fitz -- a collision to the port aft quarter indicates the other ship was very likely in the wrong, and the Straits may be fairly crowded there.
Still should have been avoidable, but it is at least theoretically possible the McCain didn't have enough maneuvering room when it got rammed.
