Why a maternity rider could cost as much as $1,600 a month for a 44 year old woman... because it was a 44 year old woman having a baby and the risks of complications was much higher than say a 20 year old woman which had a rider of only $220 a month (essentially pre-paying for the maternity ward and delivery costs with a small premium to cover unexpected complications.)
Of course, males needed maternity coverage as well, as it would be sexist to base a calculation of risk based upon such incalculable variables as someone's gender...
Contraceptives for everyone! Simply calculate what a monthly prescription would run, add a bit on top to cover paperwork and any special needs - oh, wait, are devices covered? Assume implant and removal surgery for everyone and price accordingly.
