He was talking to a reporter, who was plainly microphoned up, and crew from Access Hollywood, on a bus owned by Access Hollywood and that had "ACCESS HOLLYWOOD" plastered across the sides. Access Hollywood is some celebrity news show, and Billy Bush is some kind of two-bit entertainment reporter.

It was a "hot mic" and the reporter had his microphone on, because the reporter and Trump were waiting to step off the bus and meet some actress for the cameras.

In other words, this was just plain stupidity on the part of Trump.
