You can clearly see a wire running from the box below her waist up to her neck...the wire going to her right ear. I saw the cord live while she was bent over shaking hands after the debate, at the point Bill arrived to escort her back to center stage.
It is probably the setup for the lapel mike she had on.
If she had a comm device in her ear it would not have any wires.
