Why would you do that, if you truly possess the record and wish to disclose it, and feel sufficiently insulated that you won't be identified?
On the other hand, if you are just making it up, and don't actually know her SSN and acct #, then you'd redact same. After all, the simple sheeple are used to seeing redacted documents. And, if you made up false values, HRC would jump on that to prove the records false.

I know there is no “standard form.” I read at least 20 physician office visit documents every day prior to sedating patients for surgeries and procedures. There are certain things that are always in such a document and they are almost always grouped in the way that a MD’s are taught to analyze a patient current status. I don’t see that here.
