The problem is that the appellate judges are political animals.
Most federal judges are rejects from the DOJ and thus beholding to their administration masters.

This is a major domino to fall. These subsidies were keeping many of the exchanges tethered by a thread. Now even that will be going away. The economics just do not work. Anyone with a modicum of logic can tell you that. The infirmed will of course flock to ACA and sign up eagerly with on pre-condition restrictions. And they’re going to use a disproportionate amount of the available resources. Who can blame them?
