“Pray tell, how do we know that there have been transgendered people as long as there have been humans?”

Human biology has always resulted in people who are physically and mentally intersexed, which in turn crosses the boundaries of the social convention of assuming there are only two sexes and two genders; i.e. transgendered. For reasons of personal safety the vast majority of people who are intersexed have found it necessary to conceal their intersexed condition, even to the point where it has caused their death. For example, one such person, a hermaphrodite, was forced to live as a boy by an intolerant father. To make a long story short the person became mortally ill due to the menstruation having no physical outlet and the person being afraid to seek medical attention for the stigmatized disorder.

Human biology also results in cancer - Per Romans chapter 1, how does that usually work out for the host?

Sure, there have presumably always been a few hermaphrodites as, essentially, biological mistakes. But we really have no idea how early humans understood the sexes. I am pretty sure they would not have had “intersex” identities, however. That seems like a projection back from modern conceits.
